What is geriatric care management?

Geriatric Care Management refers to consultation and planning services provided to elders, their families, and their caregivers in response to care needs. Professionals who render these services are called Geriatric Care Managers.

Professional Geriatric Care Managers have training and experience in helping individuals and families meet the challenges of aging with dignity. The geriatric care manager helps to smooth the process of caring for the elderly family member while maintaining trust, support, and open communication among all family members.

The Geriatric Care Managers at ARCM understand the impact of aging on physical and mental health and how aging affects the ability to function well within one's environment.

Our professional care management team is trained to

  • Assist with life choices such as home care or facility placement options in order to assure the overall well-being of the individual while maintaining independence and dignity

  • Help individuals locate and use services and equipment, including diagnostic and therapeutic services, mental health services, and adaptive or assistive devices

  • Interface with public and private resources to meet the needs of the older individual as required

  • Facilitate access to federal, state, and local financial assistance programs
  • Apply their extensive knowledge of legal and medical documentation to assist families and caregivers in negotiating

    • Durable Powers of Attorney
    • Health Surrogacy
