
ARCM
provides evaluation services, counseling, referrals, and recommendations
for meeting the needs of the elderly individual. Our goal
is to maintain efficient, cost-effective, and timely delivery
of services with referrals to other healthcare providers when
appropriate.
Our
comprehensive approach to care management begins with an initial,
private telephone or in-home consultation, followed by a comprehensive
evaluation. We then develop a report outlining care concerns,
areas of immediate needed assistance, and recommendations
in the form of a care plan. The care plan is implemented after
consultation with and approval by the elderly person and the
family.
The

referrals, if needed

or other necessary services

creative plans for ransportation and appointments, meals
and meal planning, chore and household services, home
modifications, financial and banking management

on-going communication with the family and other involved
parties, providing feedback and review of the status of
the individual for continuity of care and realistic decision-making

durable medical equipment and medical supplies, and sources
for medication management
